FORM 1 REGISTRATION APPLICATION / BIRTH INFORMATION WORKSHEET

GENERAL: Complete the boxes as follows for each type of information.
Ownership of Dam- Progeny are to be registered in the name of the owner on record of the registered dam at the time of the calf’s birth. (Unless flush report for embryo calf is on record at the AHA office AND the embryo certificate is provided with this registration application).
C Birth weight information on your calf-crop is optional under the guidelines of Whole Herd TPR.  However, if you report some birth weight data, you must report for all animals of that sex from the calf-crop for it to contribute towards your herd's EPDs.  Failure to do so will prevent the birth weights submitted from being used in EPD analyses until complete information it recorded at the AHA office.
D Transfer on entry (provided at the bottom of the form) must be completed properly in order to process. For all calves that you wish to transfer, complete all information on each calf 
  
DAM’S INFORMATION:  COW HERD ID or REGISTRATION NUMBER -Provide dam’s Herd ID OR dam’s registration number on file at the AHA office.  If you are a pedigree breeder, you will have to use registration numbers.
UDDER SCORE -(Measurement taken at time of calf birth) (1=Very Pendulous, 9=Ideal) For further details, refer to the Udder Score Reference chart. 
  
SIRE’S INFORMATION: SIRE HERD ID or REGISTRATION NUMBER -Provide sire’s Herd ID OR sire’s registration number on file at the AHA office.  If you do not maintain a bull herd inventory or you are a pedigree breeder you will have to use registration numbers.
TYPE SERVICE -Mating type. (A=Artificial Insemination, N=Natural Service, E=Embryo Transfer)
  
REGISTRATION: REG CALF? -Place a “Y” in the box to register calf, or an “N” to not register calf.
  
CALF’S INFORMATION: HERD ID -Calf’s Herd Identification number (maximum 8 characters), typically the left ear tattoo.
TATTOO
Right Ear: Calf’s right ear tattoo. The lower third of the right ear on heifers should be reserved for Bangs tattoo. Avoid the ribs. (maximum 8 characters) Calf tattoo is required. It’s recommended that a “year letter” be used to indicate year of birth, with a unique number for each calf, following this system: 
2003-N, 2004-P, 2005-R, etc. The letters I, O, Q and V should not be used.

Left Ear: Calf’s left ear tattoo. Refer to instructions for right ear tattoo (maximum 8 characters).
SEX -Calf’s Sex. (B=BULL, C=Cow or Heifer, S=Steer)
CALVING DATE -Calf’s date of birth. (MM/DD/YY)
BIRTH WEIGHT -Calf’s actual birth weight, measured within 24 hours of birth.
CALV EZ -Calving Ease Score.
H/P/S -Calf's horn status. (H=Horned, P=Polled, S=Scurred)
TWIN -Multiple (twin) birth code. (1=single, 2=twin to bull, 3=twin to heifer, 4=other)
BIRTH GROUP -Dam’s birth management group. (1=MGMT Group 1, 2=MGMT Group 2, 3=MGMT Group 3, etc., maximum 2 characters) Assign a different MGMT group number for each group of dams managed differently.
NAME -Calf’s name. (maximum 30 characters) ET calves must have the last 2 characters of the name be "ET". If you do not allow for this, the calf's name will be modified to accommodate.
